Trigger Point Injections

Trigger point injections (TPI) may be an option for pain management in some patients. Trigger point injections is a procedure to treat painful muscle areas that contain trigger points, or muscle knots that form when muscles do not relax. Often, such knots can be felt under the skin. Trigger points can irritate the nerves surrounding them, causing what is called referred pain (pain felt in another part of the body).

Where can trigger points occur?

Trigger points can occur in any muscle, but the most common muscle groups treated with trigger point injections include:

  • Masseter, a muscle in your jaw
  • Levator scapulae, a muscle on the sides of your neck
  • Gluteus medius, a muscle in your hip
  • Quadratus lumborum, a deep muscle in your lower back
  • Trapezius, the muscle that extends across the back of your neck and shoulders
  • Sternocleidomastoid, a muscle at the front of your neck
  • Temporalis, a muscle on the sides of the head

Trigger points can cause certain types of pain or problems. For example, trigger points that affect the trapezius muscle can cause tension headaches. A trigger point that affects the piriformis muscle (a muscle in the buttocks) can cause piriformis syndrome, when the piriformis muscle presses on the sciatic nerve. This causes pain or numbness in the buttock and back of the leg.

What can cause trigger points?

Before your healthcare provider can recommend a trigger point injection, he or she must examine you thoroughly and rule out other possible causes of your pain, such as

  • Muscle strain.
  • Structural causes of pain.
  • Problems with the spine that cause back pain, such as degenerative arthritis, a herniated disk or spinal stenosis.
  • Radiculopathy (pain due to a pinched nerve)
